About Malmstrom Youth Programs
Youth Programs (YP) provides activities and programs that improve and maintain the physical and mental well-being of youth ages 6 to 18.
The YP facility has a full sized gym; game room with pool table, foosball, video games, snack bar and school-age care wing. We also have an outdoor play area, basketball court and three baseball/softball fields.
Malmstrom Child & Youth Programs MISSION STATEMENT
To assist DoD military and civilian personnel in balancing the competing demands of the accomplishment of the DoD mission and family life by managing and delivering a system of quality, available, and affordable programs and services for eligible children and youth birth through 18 years of age.
Membership
Youth, ages 6 to 18, are eligible to purchase an annual membership. Yearly Membership: $60 | Monthly Membership: $10. Annual memberships expire on September 30.
With a YP membership you receive discounted or free selected programs and activities.
As per AFI 34-144, participation in youth programs is limited to youth up to 18 years old. Children 5 years and under are allowed to participate only in those activities organized specifically for their group. Family members of active duty, retired military, DoD and NAF civilian personnel are allowed to participate in programs and activities. Children 6 to 8 years old must be accompanied by a parent or sibling over 16 years old unless they are participating in an organized, supervised activity.
Parents must complete an AF Form 88, Youth Programs Application, and provide a current immunization record (including the annual influenza vaccine) whether their child is participating as a member or non-member.
Youth Program Registration
1CORE: AF Child and Youth Programs (CYP) uses an online management system for program participation. Parents will create a user account and complete your family’s profile. Your profile and any other registration paperwork (i.e., a copy of current immunizations, health assessment, if applicable medication authorization, allergy exposure response, asthma action plan, emergency medication action plan, special needs package, etc.) must be completed prior to your child’s/youth’s attendance in CYP.
Note: If your child/youth has been identified with a special need, an Inclusion Action Plan must be approved prior to enrollment.
Note: If you have an existing account from another installation, please contact the front desk for account transfer. In addition to 1CORE the following needs to be completed:
- Complete Special Needs paperwork if applicable.
- Sponsor’s LES and spouse’s pay statement
- School Age Parent Agreement
- USDA Eligibility

Shot Records
*As per AFI 34-144, 11.5.3. “In accordance with AF Instruction 48-110 IP, Immunization and Chemoprophylaxis for the Prevention of Infectious Disease, all children and youth enrolled in CYP are required to follow Center for Disease Control and Prevention (CDC) Advisory Committee on Immunization Practices (ACIP) recommended age-appropriate vaccinations and immunizations. These individuals may not attend CYP without an approved medical/religious exemption from AF/A1S on file.” WHAT DOES THIS MEAN? – Any youth participating on a regular or recurring basis in Youth Programs such as: School-Age Care, sports, open recreation, Teen/Tween nights, etc. must provide a current copy of immunizations.
YP Non-Discrimination Statement
